Patients Collaborating with Teams (PaCT) takes a proactive, systematic approach to enable patients to manage their care when they have, or are at risk for having, multiple chronic diseases or other complex health needs. PaCT is inviting Primary Care Networks (PCNs) and primary care clinics to participate as Innovation Hubs. Building on the panel and screening work already underway in Alberta, Innovation Hubs will test innovative ideas to systematically support their patients with complex needs.
Expressions of Interest (EOI) to participate as an Innovation Hub closed on May 5, 2017. Currently, conversations are happening with interested PCNs. Working with a small number of member clinics over the course of fifteen months, PCNs will test evidence-based care planning approaches for identifying, planning and managing care with those patients who would benefit most from a truly shared care planning process.
