Participation Information

Participation in the Health Screen in ACT10N campaign is simple:

• Use a checklist during adult periodic health exams for one year
• Complete a small pre and post-checklist chart review to show the impact the checklist has on your
  screening process. (Note: support available from TOP upon request.)
• Submit to TOP the number of checklists used each month

Enrolment in the Health Screen in ACT10N campaign is now closed. If you have enrolled and wish to earn Mainpro-C credits for participating in the campaign, please complete the Pre-Reflective exercise [pdf]

 

Once TOP has received your information, a TOP Clinical Process Advisor will contact you to:

• Answer any questions you have about the campaign
• Discuss how the checklist will be incorporated into your practice (either on paper or within your electronic
  medical record)
• Make arrangements for a pre-checklist (baseline) chart review

Protection and Privacy
The Toward Optimized Practice Program and the Health Screen in ACT10N campaign respect the privacy of participants and are committed to protecting personal physician information. No personal patient information will be collected.

In order to track and report results to participating physicians, the campaign will collect, use and disclose information related to its participants and has adopted detailed policies in this regard. For futher information please see the Privacy Policy page.
