Improvement Facilitator Training
Who should attend?
PCN staff designated to fulfill this role. NOTE: open to clinic staff (ideally participant should be PCN staff to engage multiple clinics in this work)
What is it about?
An Improvement Facilitator (IF) plays a key, complementary role in supporting clinic teams with their panel improvement work. Panel provides the foundation for building medical homes for patients. IFs support member clinics as they take the next steps. IF training provides participants with practical skills and knowledge, including: the Patient’s Medical Home (PMH) approach, panel, quality improvement (QI) tools, facilitation skills, administering the PMH assessment, and much more. IFs are also trained in facilitating Alberta Screening and Prevention (ASaP), a provincial program for optimizing screening processes through panel management.
- 4 days, Session 9:00-4:30 (2-day blocks, 2 weeks apart)
- Facilitated by TOP
- Ongoing mentoring and support from TOP Improvement Advisors, as well as a provincial Community of Practice
- A light lunch will be provided
